Summary Of Benefits

StraCon strives to provide the best benefits to our employees. The following summary outlines the benefit options that we offer to all eligible full-time employees. Full-time employees are defined as employees working at least 30 hours per week.

Medical

StraCon offers participants a choice between PPO, EPO, and HSA plans for Health, Dental and Vision through United Healthcare. Coverages include wellness and sick visits; labs, x-rays and other types of tests; emergency and urgent care; maternity visits before and after having a baby; hospitalization, home infusion therapy, home health care, hospice care, serious mental illness and mental health care.

Coverage (should you elect this benefit) begins on the 1st day of the month following hire date.

Dental

StraCon participates in United Healthcare dental program. Under this program you may use any dentist you wish; however, out of pocket costs may be lower when you select a participating dentist. Participating dentists agree to adhere to United Healthcare processing policies and are prohibited from billing a patient above the pre-negotiated fee, accepting billing under these terms as payment in full resulting in savings to our group plan.

Coverage (should you elect this benefit) begins on the 1st day of the month following hire date.

Short-Term & Long-Term Disability Coverage

Under StraCon’s Short-Term Disability policy, you are entitled to a benefit equal to 60% of your weekly earnings up to a maximum of $1000 per week. This amount is payable to you should you become temporarily disabled for a period of at least 7 days. The cost of this benefit is paid for by StraCon.

Our Long-Term Disability policy covers you if you lose part of your income or are unable to work due to a total disability. Your benefit will begin after you have been disabled for a period of at least 90 days. If you qualify you will receive 60% of your monthly earnings up to a maximum of $6,000 per month. The cost of this benefit is also paid by StraCon

Coverage (should you elect this benefit) begins on the 1st day of the month following the first 30 days of employment.

Life Insurance

Our Life and Accidental Death & Dismemberment insurance is provided through Unum. Each full time employee will receive $50,000 of AD&D as well as $50,000 of Life Insurance. StraCon pays for 100% of the cost for this benefit.

Coverage (should you elect this benefit) begins on the 1st day of the month following hire date.

Maternity and Paternity Benefit

StraCon will pay for the first week of maternity or paternity leave. Vacation leave or leave without pay must be taken after that until the STD benefit begins.

Coverage (should you elect this benefit) begins on the 1st day of the month following the first 30 days of employment.

Supplemental Life Insurance

The employee can elect to take extra insurance coverage for up to $250,000.00

The employee’s spouse can be covered for up to ½ of the employees elected amount.

Coverage (should you elect this benefit) begins on the 1st day of the month following the first 30 days of employment.

401k Plan

StraCon will match $1.00 for every $1.00 contributed up to 3% of an employees annual salary. StraCon will then match $0.50 for every $1.00 in contributions on the next 2% of an employees annual salary.

Must work at least 1500 hours per year and have been employed for at least 90 days.

Benefit Enrollment Process

What do you need to do to enroll? Simply complete and sign the appropriate enrollment forms and return them to Human Resources. You have 15 days from the date you are eligible for coverage to submit your completed enrollment forms to the insurance carriers. If you miss this due date, you will have to wait until our next Open Enrollment Period to elect coverage. Open enrollment is held in the 4th quarter of the year for changes for the following calendar year.

Note: After your initial enrollment period, you cannot make changes to your coverage during the year unless you experience a change in family status, such as:

Loss or gain of coverage through your spouse.

Loss of eligibility of a covered dependent.

Death of your covered spouse or child.

Birth or adoption of a child.

Marriage, divorce, or legal separation.

Switch from part-time to full-time.

You have 30 days from a change in family status to make changes to your current coverage.

Paid Time Off (Vacation)

First 3 years of service = 80 hours

Years 4-6 of service = 120 hours

Years 7-10 of service = 160 hours

Years 11-19 of service = 200 hours

20 + years of service = 240 hours

Termination of Benefits

All benefits stop at the end of the month of termination.
